En español | Medicare providers (for example, hospitals, skilled nursing facilities, home health agencies and physicians who accept assignment) are required by law to submit directly to Medicare any claims for covered services that they have provided to Medicare patients.
However, there are situations in which you may be asked to pay a physician who does not accept assignment and then submit your own claim for reimbursement to Medicare, using form CMS-1490S.
