Video: Helping People Keep Their Medical Appointments

GRANTEE: Saint Agnes Foundation

The Ascension Trusted Ride Chaperone Program at Ascension Saint Agnes Hospital provides patients with round-trip, door-to-door transportation to their medical appointments. In addition to recruiting volunteer chaperones, the grant created the Chaperone Connect Project to help the participating patients overcome social isolation and gain independence.
