My Personal Health Record, South Carolina (MyPHRSC) is a Medicare sponsored pilot project that has been providing South Carolina residents with Original or Fee-For-Service Medicare in South Carolina with free access to an electronic Personal Health Record since April, 2008.
When an eligible Medicare beneficiary registers for MyPHRSC, 24 months of Medicare claims data for care received in South Carolina are pre-populated into the record. This innovative tool provides registered users with one convenient location to track their Medicare hospitalizations, diagnosed conditions, visits, medications, allergies, non-Medicare health care services, and their own notes. Users may access their information via the internet 24 hours a day, 7 days a week online . It’s Your Health. Your Record. Online, Anytime.
MyPHRSC also provides useful tools, such as; a Wallet Card that contains important information in a compact format, a Family Health History Tool which allows users to create a comprehensive family health history, a Health Tracker tool to track specific health measures, a Medication Checker which compares medications for potential medication conflicts, links to useful websites for health-related information, and a News feature with articles about specific health topics.
Additionally, through a simple process, users can authorize representatives, such as a provider or family member, to have access to their PHR. Users may choose to give these authorized representatives full or limited access to their record
South Carolina residents with Original or Fee-For-Service Medicare can create their free PHR at any time on the web.
