To answer your questions......first, if they went to the ER under Medicaid, then they are already insured and will not be affected by the ACA.
. . . . Second, the people without Medicaid will get it if their income is under a certain percentage of the poverty level (I beieve it's 137%) and it will be free. A caveat to that though is getting covered under Medicaid will depend on whether or not their state is participating in the Medicaid expansion.

Sorry, Quacker but there is a footnote here.
Many states had their current Medicaid programs configured to cover people with incomes higher than what was legislated under the Federal Medicaid entitlement. These people, under these special state specific Medicaid plans, will the removed from the current Medicaid rolls of their state and they will have to seek coverage in another way - they will either go to the exchange and pick a plan or they will have to enroll in the expanded medicaid program if available in their state.
Also, I don't believe that the expanded Medicaid will be FREE in all participating state cases. I have read that some of these states are looking or expanding their Mediciad managed care plans ran by private insurance companies and that there maybe some co-pays that are applicable for certain thing.
