I am wondering if anyone can share their experience with me regarding Long Term Care Services today? And then how Health Care Reform implementation may influence Long Term Care Services for yourself or a loved one in 2014? Are there any concerns on the horizon as the ACA starts implementation by 2014?

I am a Healthcare provider assessing how such policy may influence unserved populations and special populations. I am somewhat involved with Health Policy at the State and Federal level, and I am very interested in hearing people's understanding of Health Care Reform, it's potential affect on you, and benefit or concerns in the future with respect to Long Term Care and the ACA.

I look forward to hearing or reading responses, and stories..

The only thing that I can recall in the ACA that will affect a type of LTC will be the expansion of Home and Community Based Care for those under the Medicaid program.  This will not only cover some medical monitoring but personal care and other service too, unrelated to medical.

Once these folks cannot live in their own home, I think there will be an expansion of community (residentail) based care for them, like personal care homes or whatever the various states call them. 

This leaves SNF for more rehab and those folks who need the LTC extreme of medical care.  It is kind of crazy to put a person on Medicaid in a SNF when the majority of their needs are personal care so options have to remain open to other type facilities.

I am paying out of pocket for my mother's care.  She is total care.  The cost is $ 3500 a month - but that sure beats paying out of pocket for a SNF, which around here is about $ 7500 per month.  There are (6) residents in the home, all are self paying and this is the max number they can take based on the size of the home.  This is a great place and where she needs to be AND it is more like a home than a SNF.

However, I do think that people should be strongly encouraged to complete a Health Care Directive or Living Will, before it is too late to make that decision for ones self.  Perhaps it should be a stipulation that they have one before going under any LTC plan.

My mother would never complete an official one but she covered the basics in a hand written note back in 2006.  I have had no one question it especially being in the shape that she is now in. 

SNF are having some of their reimbusements cut and so are DME - this would be under Medicare and the rehab which they perform rather than LTC but it is all a part of the bottom line for them.

I think there will be a big opportunity for employment within this area especially with all us baby boomers moving in that (aging) direction.  If you are really good at what you do, from Level One care givers up , I think you could draw a good return but only if self employment or contract work is the negotiating method.  If you are working for an agency, especially one that is non-profit and serving Medicaid clients, it will be hard to make a good wage since cost will be watched sharpely.

I thought you might find this GAO Report to the Congressional Requesters in July 2012 interesting.

MEDICAID LONG-TERM CARE Information Obtained by States about Applicants’ Assets Varies and May Be Insufficient

We have got to start changing a few things ASAP.
