In Faces of Chronic Care, patients and their caregivers offer a personal look at the daily struggle to deal with chronic illness and conditions.
The AARP Public Policy Institute created this video as a companion piece to the PPI report, Chronic Care: A Call to Action for Health Reform, which examines problems the health care system faces delivering care for many older Americans. The 2009 study concludes that addressing barriers to providing better chronic care requires a multi-pronged strategy that relies on better knowledge, tools, and incentives.
