What Alzheimer's "looks like" varies as much as the people it affects. For some, the vibrancy and achievements of their pre-Alzheimer’s lives are quickly eclipsed by the challenges of the present. In other cases, a person's connections with family and friends – and with his or her own sense of self – don't change all that much for at least a few years.

AARP recently featured photographer Gregg Segal’s compositions of people with Alzheimer's, as they appear today, and in their earlier days. "Memories may fade," we noted in our introduction to Remembered: The Alzheimer’s Photography Project, "but the beauty of a life well-lived never dims."

If Alzheimer's has touched your family, AARP invites you to honor your loved one in pictures as well.

The photographs and information you share will be considered for inclusion in a slideshow here at AARP.org. You may submit two images (one from the past, one more recent). To do so, please complete the form below, hit the "Submit" button and follow the instructions on the page that appears for how to upload your pictures.
