You can obtain a form to create your living will from your local hospital, or you can download a form specific to your state from the websites of the U.S. Living Will Registry and the Partnership for Caring. You also can create your living will using an online tool or software. Additionally, many of these tools help you create a durable medical power of attorney. Here are a few options:
